A complete application must include the following:

_____ Completed and signed application form submitted on or before May 15, 2011.

_____ Official course transcripts for pre-enrollment courses. Note: If the course is "in progress," transcripts must be submitted as soon as they are available.

_____ Official verification of equivalencies where course substitutions have been made (see counselor).
